Hello and
welcome! We are pleased that
you have chosen our office for your dental care needs.
The following information is intended to
acquaint you with our practice so that we can establish a comfortable
and
successful relationship. Since the most
important service we can offer you is a diagnosis, we pride ourselves
on our
thorough examinations. It is critical
that we know what is normal and what is abnormal in your mouth, but
more
importantly, why things are the way they are. We
will carefully study your dental and medical
history, x-rays and
study models and review the clinical examination findings to accurately
predict
your dental future.
The
health and appearance of your
mouth is very closely associated with your overall health and
appearance. Faulty appearance and function
can create
problems affecting both your physical and emotional well-being. Therefore, in order for us to effectively
diagnose your oral health (as it relates to your general well-being),
we

A few days after the
first and
second appointments a Diagnostic Report will be prepared for you and
given to
you at your “New Patient Consultation” time. Your
written personal diagnostic report is a
comprehensive review of
your examination findings. While you
are reading this report, questions may arise in your mind and we
encourage you
to make note of them so that they may be fully discussed.
At your Consultation Appointment, you will be
able to sit down with your dentist one-to-one to discuss your study
models,
x-rays, diagnostic report and the results of your examination. Your dentist will work with you to develop a
treatment plan that will suit your needs and achieve the results you
want for
yourself. Of course, we will always
discuss your treatment options where possible, and provide you with a
